Community Action Plan

The Kent County Infant Health Initiative with input from the community developed the action plan to address the disparities between African American and white infant survival. The plan outlines the goals and objectives that have become the foundation for the work of the Kent County Health Department and Healthy Kent 2010's Infant Health Implementation Team.
